Tying an answer to something real instead of to the model's own fluency.
In practice: making the system fetch the actual policy, the actual balance, the actual record, and answer from that. It is the single most effective technique against confident invention, and it is why a system with access to your data behaves very differently from one that is only guessing well.
Why it matters
Most reliability problems are access problems wearing a costume.
